How to Enroll & Disenroll
To start or end your membership in CareAdvantage, you need to complete certain forms and send them to the CareAdvantage Unit. If you have questions about these forms or the enrollment and disenrollment process, call the CareAdvantage Unit and a Navigator will help you.

How do I enroll?
If you are eligible for CareAdvantage, you can choose one of these ways to enroll:
| Call the CareAdvantage Unit and ask to have an Enrollment Form mailed to you. You can also download the Enrollment Form. Complete and send it to the CareAdvantage Unit.

| Or you can apply online through the official Medicare enrollment website, by going to the CMS Online Enrollment Center (OEC). |

How do I disenroll?
If you decide to leave CareAdvantage, you must tell the Health Plan of San Mateo (HPSM) or Medicare about your decision.

Fax or mail a Disenrollment Form to the CareAdvantage Unit. Be sure to sign and date your form. If you fax this form, you still need to send the original copy.

| HPSM will send you a letter that tells you when your membership will end. This is the disenrollment date. You must continue to receive all medical care from CareAdvantage providers until the effective date of disenrollment. Contact us to verify your disenrollment date before you seek medical services outside of CareAdvantage’s network. |

Who else can I call for questions about my rights?

You can call the national Medicare helpline, 1-800-MEDICARE (1-800-633-4227), 24 hours a day, 7 days a week. TTY users should call 1-877-486-2048.

You can call the San Mateo Health Insurance Counseling and Advocacy Program Office (HICAP). They provide free counseling to Medicare beneficiaries, disabled adults, seniors and their families or other representatives on how the health care system works and how to get services. They can be reached at (650) 627-9350 or 1-800-434-0222. |
