Prenatal Care: Receive Gift Cards
We will reward you for going to your prenatal appointments to help keep you and your baby healthy. You will receive rewards when your doctor confirms that you have gone to your prenatal care visits.
$15 Target gift card
 After 1st prenatal visit within first 12 weeks of pregnancy
A book, "What to do When You are Having a Baby"
After 3 months of confirmed prenatal visits

Living Healthy Women’s Program (Pap test and Mammogram): Receive Gift Cards
We will reward you for getting a Pap test and for getting a mammogram.

  Offer a support person to go with you to your appointments for
    emotional support when you have tests to screen for cancer.

Annual Teen Check Up: Get Two Free Movie Tickets
We want our young members between the ages of 12 and 18 to see a doctor once a year for a free checkup.
Two free movie tickets to Century theaters
HPSM will mail movie tickets within one month of receiving the HPSM movie tickets form with a doctor’s signature, confirming that teen member has received a check-up.
Schedule an appointment with your teen’s doctor.
  Print the HPSM Two Free Movie Tickets form
  Complete the form and have your doctor sign it.
