Whole Child Model
HPSM’s Medi-Cal program includes a special plan called the Whole Child Model that is for children under 21 years
of age with certain diseases or health problems.
Whole Child Model eligibility
If you have a child who is eligible for the program, HPSM will notify you. If you or your PCP believes your child is eligible for program, fill out an application.
