What is the HomeAdvantage program? 

HomeAdvantage gives free home-based medical care to select members of HPSM’s CareAdvantage program. HomeAdvantage care teams include doctors, nurses and other specialists. They can make “house calls” to your home for check-ups and treatment. They also coordinate with your primary care provider. The HomeAdvantage teams are available 24/7 by phone and can make “house calls” in emergencies. Learn more about HomeAdvantage.

What’s changing, and why? 

The HomeAdvantage program and the services are not going away. The provider group that delivers these services will be changing from Landmark Health to Upward Health. HPSM has worked with Landmark since 2016 on the HomeAdvantage program, and they delivered excellent services to our members. HPSM thinks that Upward Health will be a better fit for CareAdvantage members. Upward Health will deliver the same services you have received in the past. We want to make sure that you will have a smooth transition to Upward Health.

Another change is that HomeAdvantage will offer three different service tiers to better meet different members’ needs. Learn about the three tiers.

When will transitions to the new provider start? 

Upward Health will start to see CareAdvantage members in January 2021 and other members in February or March 2022. Upward will be completely responsible for program and patients as of March 1, 2022. You will be receiving additional details closer to the date of your transition.

What additional information will I receive about this change? 

HPSM will send you a notice as you approach your transition date. You will also get an outreach call from your new Upward Health provider in the month before you transition to their care. Upward Health will reach out to you in December, January or February (depending on when you are scheduled to transition). 

When will I learn more about Upward Health?

HPSM will send you more information about Upward Health in the next few months and as you approach your transition date. 

How do I know if I am in the HomeAdvantage program today? 

You will receive a notice from HPSM about the HomeAdvantage transition.

Will my home-based provider change?

Yes. Your new Upward Health provider will have information about your current care plan. They will know what care you have received. Your care will not be disrupted. Your new provider will get to know you and provide services that meet your current care needs.

I like my provider and don’t want them to change.

We are glad you like your current home-based provider. We do not know if you will be able to keep that provider. If you know that provider’s name and information we can share that with our internal team. We want you to have a good relationship with your new provider. Your Landmark provider will share your specific health needs and preferences with your Upward Health providers.
