CareAdvantage Cal MediConnect Plan (Medicare-Medicaid Plan)
CareAdvantage CMC covers brand and generic medications that are:
- Prescribed by your doctor
- Taken by yourself at home
- Given to you at a doctor's office, hospital or skilled nursing facility
- Given to you in the form of injections related to dialysis and chemotherapy
There may be limitations on the types of drugs covered. For more information, see the CareAdvantage CMC Formulary (list of covered drugs), which includes all of the drugs CareAdvantage CMC covers.
CareAdvantage Drug Coverage Restrictions
Prior Authorization Criteria (PA)
These drugs are in the Formulary but need to be pre-approved before you fill a prescription. This also includes the criteria for approval of each drug.
Quantity Limits (QL)
These drugs are covered but have a set limit on the amount of pills per prescription and the number of times they can be refilled without further authorization.
Step Therapy Criteria (ST)
Some drugs are covered only after you have a tried a recommended drug, but it did not work. When there are two drugs for your condition, CareAdvantage may require you to try one drug first before covering the other drug.
Drugs Not Covered
By law, there are categories of drugs that Medicare Part B and Part D, and Medi-Cal, cannot cover. You can still get drugs that are not covered, but CareAdvantage will not pay for them. You must pay for the full cost of drugs not covered.

The CareAdvantage CMC Formulary Updates show;
- Drugs that have been added to or taken off the list
- Brand drugs that have been replaced with generic drugs
- Changes in coverage restrictions
CareAdvantage Cal MediConnect Plan (Medicare-Medicaid Plan) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees.
