CareAdvantage Cal MediConnect Plan (Medicare-Medicaid Plan)

Eligibility

To join CareAdvantage CMC and remain a member, you must:

  • Live in San Mateo County,
  • Have Medicare Parts A & B and
  • Have full-scope Medi-Cal.

Live in San Mateo County

If you move out of San Mateo County, you cannot stay in this plan. To find out what your other health care options are, call your local county eligibility worker at 1-800-223-8383 Monday through Friday between 8:00 a.m. and 5:00 p.m.

Medicare rules do not allow you to live outside of San Mateo County for more than six months in a row. If you are away longer, you will lose your CareAdvantage coverage. To keep your CareAdvantage CMC plan up-to-date, call the CareAdvantage Unit.

Medicare and Medi-Cal

CareAdvantage CMC is for people who have both Medicare parts A & B and Medi-Cal. If you lose your Medi-Cal for some reason, you must get it back within two months in order to remain a CareAdvantage CMC member. If you are no longer eligible for Medicare parts A &  B, your CareAdvantage CMC coverage will end.


CareAdvantage Cal MediConnect Plan (Medicare-Medicaid Plan) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees. Benefits and co-pays may change on January 1 of each year.
