Whole Child Model

The Whole Child Model is a partnership between HPSM and San Mateo County to deliver coordinated care and services to eligible children with complex medical conditions. It was known as the California Children’s Services Pilot until 2018.

Who is eligible

Children under 21 years of age with certain diseases or health problems.

What is covered

HPSM’s Whole Child Model can help pay for these and other services and treatments (if they are deemed medically necessary):

  • Doctor/specialist visits
  • Surgeries
  • Hospital stays
  • Lab tests and x-rays
  • Medicine
  • Diapers
  • Mental health services
  • Medical supplies
  • Orthopedic appliances
  • Medical equipment (e.g., wheelchairs, hearing aids, etc.)
  • Physical, occupational and speech therapy
  • In-Home Nursing Services (IHSS)

Cost

The Whole Child Model is part of HPSM’s Medi-Cal program. There is no extra cost for the Whole Child Model. If your child is covered under Medi-Cal, HPSM will continue to pay for the services that your child needs, regardless of whether your child qualifies for Whole Child Model.

Apply

If you have a child who is eligible for the program, HPSM will notify you. If you or your PCP believes your child is eligible for program, fill out an application.
