Why Whole Person Care is needed

San Mateo County has many resources that can help people. But they too often work in isolation from one another, so case managers in one organization don’t know what other services their client is already utilizing. This can result in duplication of services, with people seeing two or more providers for the same service, which wastes finite system resources. It can also lead to the opposite problem: people falling through the social safety net because providers don’t know what services they are not getting. 

Whole Person Care solves these problems by providing personalized one-on-one mentoring that teaches people how to use services and get what they need. It also coordinates providers’ efforts so they know more about the other services people are receiving, making a truly integrated team treatment approach possible.
