CareAdvantage members assigned to PAMF for primary care are invited to join Grove San Mateo. At Grove, a primary care team of PAMF and HPSM clinicians will take care of you like we would our own family members.
It means more personalized care, not more medicine — all at no cost to you. Each team member uses their special skills to take care of your different needs. We want to be your true partner in health by providing high-quality care and
support for you and your caregivers.
What you get with Grove
- Access to all specialists at PAMF because you’ll still be a PAMF member after joining Grove.
- A dedicated care team made up of different specialists who work together, creating a care plan that meets your unique needs.
- On-demand health care appointments, including same-day, urgent and home-based visits. (You can get house calls if you are homebound or have extreme difficulty getting to the clinic.) Plus you’ll get the time you need with the team.
- Calls answered by a care team member, not a call center — so you will be speaking to someone you are already familiar with.
- All your care at one clinic conveniently located at 50 South San Mateo Drive in the city of San Mateo. This saves you travel time when you need multiple services. Use your ride benefit to get to appointments and back home.
Better health with team-based primary care
People with more complex, chronic health conditions need more personalized, coordinated care to be healthy. With Grove, you get a primary care provider (PCP) who leads an entire care team of specialists.
- Grove team includes a geriatrician, an internal medicine doctor, a social worker, and care coordinator.
- Grove team members spend as much time with you as you feel you need, and never rush your care.
- To make sure you are not missing any services or supports, Grove team members check in with you regularly. This prevents chronic disease and disability so you can enjoy more years of good health.

CareAdvantage Dual Eligible Special Needs Plan (D-SNP) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees.
