CareAdvantage members assigned to PAMF for primary care are invited to join Grove San Mateo. At Grove, a primary care team of PAMF and HPSM clinicians will work with you to understand and respond to your individual needs. This can better help you reach your health goals with:

• Access to all PAMF specialists because you’ll still be a PAMF patient after joining Grove.
• A group of specialists who provide coordinated care.
• On-demand health care appointments, including same-day, urgent and home visits. (You can get home visits if you are homebound or have extreme difficulty getting to the clinic.)
• Calls answered by someone on your care team, not a call center.
• All your care at one clinic located in the city of San Mateo.

There is no cost for joining Grove. To get started or learn more, just do one of the following:
