HPSM’s 10 Community-Based Adult Service (CBAS) centers in San Mateo, San Francisco and Santa Clara counties offer health-focused daycare programs for adults with medical, cognitive and/or functional limitations. HPSM Medi-Cal and CareAdvantage members enrolled in CBAS receive onsite health, therapy and social services at no cost. Referring your patients can help them avoid unnecessary institutionalization in long-term care facilities and live independently at home or with family.
We recently added Burlingame Adult Day Health Care to our CBAS network
You may want to recommend this CBAS center to eligible HPSM members who live in or near Burlingame and:
- Don’t use CBAS because they think other centers are too far away.
- Currently attend another CBAS center that is farther from their home. (Be sure to emphasize that they can continue going there if they wish.)
- Primarily speak Spanish, Arabic, Tagalog, Korean, Farsi or Mandarin/Cantonese, and the CBAS center they attend does not have staff who are fluent in their preferred language.
- Want to learn more and get a tour of this new CBAS center.
To refer an HPSM member to CBAS, either:
- Find a CBAS center in HPSM’s network near their home, then give the member (or their family member or caregiver) their phone number and tell them to schedule a tour.
- Fill out the CBAS Referral Form and fax it to 650-829-2047.
