Introducing HPSM’s New Primary Care Practice Grants

Starting April 15, 2025, primary care practices contracted with HPSM can apply for our four new Primary Care Provider Grants! 

Overview of Grants

  • The Primary Care Team Expansion Grant is for hiring and integrating new interprofessional team members. The funding provides the upfront dollars to hire, operationalize and sustain new interprofessional team member positions that will increase primary care team capacity. 
  • The Core Team Stabilization Grant is for recruiting and retaining primary care providers (PCPs) and medical assistants. This funding helps practices stabilize their primary care teams by offering competitive sign-on bonuses to augment compensation packages for PCPs and medical assistants. 
  • The Provider Sabbatical Grant is for retaining providers by enhancing resilience through sabbatical. This funding rewards long-term PCPs for their service and commitment to HPSM members with paid time off. 
  • The Custom Pilot Grant is for developing pilots and programs that improve primary care teams’ capacity, bandwidth and joy.

Why Invest In Primary Care? 

Primary care is a common good. Systems that invest significantly in primary care achieve better, more equitable health outcomes and enhanced quality of care. Yet, primary care continues to face chronic underinvestment, workforce shortages and provider burnout, making it increasingly difficult for individuals to access the care they need.

To transition our network from crisis to progress, HPSM is committing $60 million over the next five years to transform primary care in San Mateo County. One of the key objectives of this investment strategy is to strengthen a diverse primary care workforce that has capacity, bandwidth and joy. Our Primary Care Provider Grants are designed to support this important goal.
