Breathe California is HPSM’s New Asthma Program Partner

We now contract with Breathe California to provide our qualified members with an Asthma Program featuring:

1. Asthma preventative services, which are a Medi-Cal benefit and include clinic-based and home-based education for asthma, its triggers and causes, how to prevent attacks, symptom management, and more. 

2. An in-home assessment that helps members live more comfortably in their homes by:

  • Identifying environmental conditions that can trigger asthma symptoms.
  • Making remediations (such as removing mold and increasing ventilation).
  • Offering remediating supplies (such as air filters, vacuum cleaners, dehumidifiers, and hypoallergenic mattress and pillow covers). 

Eligibility

Eligibility is limited to individuals who meet both of these criteria:

1. Patient has been determined to have poorly controlled asthma by one of the following methods:

  • An emergency department (ED) visit
  • Hospitalization
  • Two sick or urgent care visits in the past 12 months
  • A score of 19 or lower on the Asthma Control Test  

2. A licensed healthcare provider has documented that the program will likely help the patient avoid asthma-related hospitalizations, ED visits or utilization of other high-cost services.

We provide this program through the CalAIM initiative’s Community Supports (CS) services.
