Our new Care Gaps Closure P4P program is an exciting alternative that improves patient outcomes through a customized, user-friendly data-sharing process.
Care Gaps Closure P4P features a new interactive tool that you and your staff can securely log into online. That makes it easier to get detailed data on each of your HPSM patients’ care gaps (rather than downloading individual HPSM legacy P4P reports) and report actions you’ve taken to close them.
How Care Gaps Closure P4P works
- Accrue points across the entire care continuum — first for scheduling appointments with patients who have care gaps, then for delivering services that bridge those gaps.
- Eligible services include well-visits, initial health appointments, preventive screenings, immunizations and more.
- Points are converted to funds sent directly to your practice at the end of each month.
