Diabetes Prevention Program

Do you have prediabetes? Lower your risk of getting Type 2 diabetes and improve your health by joining HPSM's Diabetes Prevention Program (DPP)! This support program is available to eligible HPSM members at no cost. 

What you get

  • Weekly one-hour sessions run by trained Lifestyle Coaches for the first six months and monthly one-hour sessions in the last six months. Sessions focus on:
    • Understanding food groups and food labels
    • Different types of physical activity and how to create an exercise routine
    • Recognizing barriers to healthy behaviors
    • How to stay motivated and create a plan for your goals
  • Educational materials to help you meet your health goals.
  • Peer support from other group participants.

How do I know if I have prediabetes? Prediabetes is when your blood sugar levels are higher than normal, but not enough for a diabetes diagnosis. If you are unsure, check in with your primary care provider (PCP). Learn more at the Centers for Disease Control website

Who is eligible

HPSM Medi-Cal members who meet all of the following:

  • Are 18 years old or over
  • Are overweight (Body Mass Index of at least 25 or at least 23 if self-identified as Asian)
  • Are not pregnant
  • Do not have diabetes (type 1 or 2)
  • Do not have end-stage renal disease (kidney failure) 
  • Meet one of the following: 
    • Have prediabetes: your PCP can tell you if you have prediabetes 
    • Have had gestational diabetes in a past pregnancy
    • Get a result of high-risk for type 2 diabetes on the CDC Prediabetes Risk Test

HPSM CareAdvantage members who meet all of the following:

  • Are 18 years old or over
  • Are overweight (Body Mass Index of at least 25 or at least 23 if self-identified as Asian)
  • Are not pregnant
  • Do not have diabetes (type 1 or 2)

Additional Services

Eligible Medi-Cal members who enroll in DPP can also enroll in HPSM’s Fitness Membership Program at no cost while enrolled in DPP. That gives you access to YMCA locations throughout the counties of San Francisco, San Mateo and Santa Clara.

CareAdvantage members can enroll in our Fitness Membership Program at no cost and use this benefit even when not enrolled in DPP. Learn more.

Transportation

DPP-eligible members can use HPSM's Ride Benefit to get to in-person DPP sessions.

HPSM's DPP is approved by the Centers for Disease Control (CDC)
