Supporting the health of our members
The HPSM Population Health Management (PHM) Program focuses on improving the health of our members. It offers five no-cost support programs to help members reach their health goals.
Baby + Me Program supports HPSM Medi-Cal members who are pregnant or recently had a baby. Participants can earn up to $100 in Target GiftCardsTM* for going to two visits!
Diabetes Prevention Program (DPP) helps eligible HSPM Medi-Cal and CareAdvantage members lower their risk of Type 2 diabetes. This 12-month program includes one-hour online or in-person group sessions led by trained Lifestyle Coaches. Participants learn to create plans for eating healthy and getting more exercise.
Care Management Program is available for all HPSM members. Participants get to work with a Care Manager on a personalized care plan based on their health goals. Members learn about self-care, useful resources, managing complex medical conditions and addressing urgent medical problems.
HPSM’s Asthma Management Program from Breathe California provides support for members with asthma. Services include education and home visits (if needed). If a home visit suggests that supplies (air filters and dehumidifiers) are needed, Breathe California can help get them at no cost.
Care Transitions Program provides the support of a Care Manager to members returning home from the hospital.
HPSM Medi-Cal, CareAdvantage and HealthWorx HMO members get doula services at no cost. Services include support:
- Before, during and after birth
- For a miscarriage
- For abortion visits
