Care Planning & Management Benefit 

Formerly known as the Multipurpose Senior Services Program (MSSP)

HPSM’s Care Planning and Management (CPM) benefit provides home-based support that improves people’s quality of life by enabling them to live at home and in the community for as long as possible. Working with the client and his or her care team, CPM provides:

• Home-based case management 
  
• Caregiver support 
  
• Care coordination that gets all the client’s providers working together as a team 
  
• Links to health and social services to optimize existing support systems 
  
• Personalized wellness planning and health education
  

Eligibility

You may be eligible for CPM if you meet the following criteria:

1. Are an HPSM Medi-Cal member 
   
2. Are 18 years of age or older 
   
3. Need case management services
   
4. Are eligible for and willing to participate in case management 
   
5. Have an outstanding physical, developmental or psychological need that requires regular assistance
