We frequently refer our members to our community partners for the vital support services they provide – such as advice, advocacy and help accessing government benefits. All of these partners strengthen our ability to ensure our members have the support they need to be healthy.
Our community partners include:
- Aging & Adult Services provides homecare services to eligible Medi-Cal members
- Behavioral Health & Recovery Services addresses mental illness and substance use issues
- Behavioral Health & Recovery Services: Chinese Health Initiative empowers people to make good health decisions
- Bridges to Wellness embraces “total wellness” – including medical, emotional, spiritual, financial and social health
- Brilliant Corners finds affordable community housing for people who are homeless, living with disabilities or living in institutions
- Community-Based Adult Services provides daycare programs that support independent living
- Deaf & Disabled Telecommunications Program enables deaf and disabled Californians to use the phone
- Health Insurance Counseling & Advocacy Program of San Mateo County (HICAP) educates people about their health care rights
- Matrix Medical Network provides in-home health visits for eligible CareAdvantage members
- Human Services Agency / CalFresh provides nutrition assistance to people with low income
- Institute on Aging helps aging adults and people living with disabilities age gracefully
- In-Home Supportive Services (IHSS) delivers support services to Medi-Cal eligible individuals with disabilities to help them live at home
- Integrated Medical Assisted Treatment Program (IMAT) treats substance use disorders by combining behavioral therapy with medication
- Legal Aid Society of San Mateo County helps low-income clients with civil legal issues
- Lucile Packard Children’s Hospital is a nationally ranked hospital that specializes in pediatric and obstetric care
- Mills Peninsula Senior Focus Center is an adult day health care centers for people who cannot be left alone during the day due to health care needs or disabilities
- Ombudsman Services of San Mateo County advocates for the frail and vulnerable residents in the long term care facilities
- Peninsula Family Service helps people realize their full potential and lead healthy, stable lives
- San Mateo County California Children’s Services (CCS) helps children and young adults up to 21 years old who have certain medical conditions
- San Mateo Medical Center provides physical and mental health care services at multiple clinic locations
