Special Programs

HPSM’s special programs focus on our most vulnerable members: older adult members and members with disabilities. Many are conducted as collaborations with our community partners – other local organizations whose services complement our own. These programs let us test innovative new ways to enhance people’s health and well-being. When programs succeed, we expand them so more HPSM members can benefit. Effective programs also serve as models that other health care organizations throughout the region and the state can emulate.

Our special programs include:

  • The COVID-19 Centers of Excellence (COE) initiative, a partnership program with San Mateo County Health (SMC) to ensure that local skilled nursing facilities (SNFs) can effectively treat COVID-19 patients in isolation from their other residents.
  • COVID-19 Outreach designed to help our members cope with pandemic-related stresses. Our three-pronged approach complements the information on our COVID-19 micro-website for members and includes:
    • Welfare phone screening to see if people need help getting health care services, nutrition assistance or some other type of support
    • Social outreach calls to reduce social isolation, which has worsened due to social distancing and sheltering in place
    • “Dear Neighbor” postcards on which community volunteers write personalized messages to potentially isolated HPSM members 
  • HomeAdvantage, which provides eligible CareAdvantage members with routine home visits, urgent on-call treatment, 24/7 phone access to providers, transition support after a hospital visit and more. 
  • The Whole Child Model and California Children's Services Program (CCS), a partnership between HPSM and San Mateo County to deliver coordinated care and services to eligible kids with complex medical conditions.
  • Wider Circle, which helps older adults make new friends and develop healthy habits by attending fun social events in their community.
