In 2014, an automobile accident left Eric confined to a wheelchair. When he could walk again, Eric was enrolled in the Community Care Settings Program, which helped him find a subsidized apartment and gave him the home-based support he needed to live on his own. Read Eric's Story

Jennifer was addicted to meth, but when she became pregnant, she vowed to get sober. She couldn't afford an apartment and was still living in a shelter two days before her baby was due. Kojo earned his law degree and was studying to become an attorney when he had a psychological breakdown. He was then homeless for the next decade. After suffering a stroke, Kojo was hospitalized and enrolled in the Community Care Settings Project. Now he has the medication, health care and housing he needs. Read Kojo's story

The Great Recession of 2008 put Marcia’s Daycare out of business after 22 years, and her family was evicted. Marcia lived out of her car while her daughters stayed with friends. Then Marcia was enrolled in Whole Person Care, which found the family a new home. The program also got Marcia the medical care and social support resources she needed to get back on her feet. At age 55, a stroke left the right side of Marijo’s body virtually paralyzed. From then on, she could no longer work – instead, she needed someone to take care of her. Then Marijo started getting regular home visits through HomeAdvantage, which brings health care to people at home whenever they need it. Read Marijo's story

In 1972, Paul got shot while being robbed, triggering a series of lifelong health problems. When he was laid off due to his injuries, Paul lost his health insurance. Nearly three decades later, the Affordable Care Act’s Medicare expansion program made him eligible for health care. HPSM then covered Paul's health care from our partner, San Mateo Medical Center. After having a stroke in 2012, Rafael could only get around using a walker. Medical bills left him and his wife Zenaida struggling financially, so they lived in a rented garage. HPSM’s Community Care Settings Program got them an apartment in a subsidized senior community that was much more affordable. Rick was hospitalized for sepsis and a blood clot in his leg, then diagnosed with several other chronic health conditions, including diabetes, Parkinson’s disease, glaucoma, an enlarged prostate and an atrial fibrillation.
