HealthWorx HMO
Where Can I Get Care?
San Mateo County is the designated service area for the HealthWorx HMO program. Service Area means the geographic area served by HPSM and approved by the State of California Department of Managed Health Care (DMHC). It is important that you see doctors
who participate with HPSM.
Telemedicine
Doctor's visits by phone or video anytime
Many providers can give you advice over the phone or see you by video. Telephone and video visits are fully covered by HPSM. Call your primary care provider (PCP) to check if they provide telephone or video visits.
If you do not have a PCP, or if your PCP does not offer phone or video appointments, you may be able to use Teladoc®, one of HPSM's newest service partners. Learn more.
Continuity of care
Under some circumstances, HPSM will provide continuity of care for new members who are receiving medical services from a non-participating provider, such as a doctor or hospital, when HPSM determines that continuing treatment with a non-participating
provider is medically appropriate based on your condition. See the HealthWorx HMO Evidence of Coverage for
more details. To request continuing care or get a copy of our Continuity of Care policy, please call Member Services. If your providers do not join the HPSM network by the end of 12 months, you will need to switch to providers in the HPSM network.
Search for a provider
Use the online provider search form to find the latest information for in-network primary care providers (PCP), specialists, pharmacies, clinics or hospitals. This directory is updated weekly. You may need a referral from your PCP before making an appointment with a specialist for non-emergency care. Call your PCP to find out if a referral is required.
For more information about where you can get care, refer to section 3 of the HealthWorx HMO Evidence of Coverage. To order a printed EOC, email CustomerSupport@hpsm.org or call Member Services.
