HPSM programs and services

As an HPSM member, you have access to several programs and covered services that can help support your diabetes management. If you have questions about these programs and services, call HPSM’s Health Promotion Unit at 650-616-2165.

  • HPSM’s Fitness Membership Program: CareAdvantage members can enroll in our Fitness Membership Program to get a YMCA gym membership at no cost to you. Learn more.
  • Medically Supportive Meals: For Medi-Cal and CareAdvantage members with certain chronic conditions. This service helps eligible members achieve their nutrition goals at critical times. Members may be eligible for up to two meals per day for 12 weeks. Learn more.
  • Registered dietitians: Get expert help creating plans for eating healthy. HPSM has in-network registered dietitians who can offer advice on nutrition and healthy eating habits. To learn more:
    • Medi-Cal members call 1-800-750-4776 or 650-616-2133 Monday–Sunday 8:00 a.m. to 8:00 p.m.
    • CareAdvantage members call 1-866-880-0606 or 650-616-2174 Monday–Friday 8:00 a.m. to 6:00 p.m.

  • Diabetes supplies: Talk with your healthcare provider to see if you need to check your blood sugar or blood pressure at home. They can issue a prescription and fill out any paperwork.
  • HPSM’s Diabetes Prevention Program (DPP): If you have prediabetes, HPSM’s DPP can help you lower your risk of getting type 2 diabetes. This program is available to eligible HPSM Medi-Cal and CareAdvantage members at no cost. Learn more.

  • Mariposa Program: no-cost annual passes to San Mateo County Parks for eligible individuals
