for People with Disabilities

HPSM partners with many community organizations to provide programs for our members, including those with disabilities. Check out our programs below and click the link for more information.

Community Based Adult Services (CBAS)

HPSM works with nine different CBAS Centers to provide daily care to older adults and persons with disabilities in the form of meal support, social activities, nursing care, and physical, speech and occupational therapy.

In-Home Support Services (IHSS)

This program provides services to older adults and persons with disabilities who have functional limitations and are currently living in their own home. Services include help with prepping meals, bathing, dressing, laundry, wound care and protective supervision. Referrals for IHSS can be made by calling the Hotline at 1-800-675-8437 or Aging and Adult Services at 650-573-3900. To apply for IHSS, please fill out the referral form.

Care Planning and Management (CPM)

This program provides case management and the purchase of services to support older adults and members with disabilities living in the community with the goal of avoiding institutionalization.

CalAIM Community Supports

CalAIM’s Community Supports services covers non-medical help to address life’s challenges. These include home safety improvements, medically-tailored meals and help transferring from a care facility to independent living.

HomeAdvantage

This program for CareAdvantage members includes three service tiers based on member’s needs. Services include live 24/7 phone support, urgent visits at home, yearly home health exams, post-discharge support and care follow up.

Wider Circle

This program helps older adults make new friends and build healthy habits by going to social events in their community.

Friendship Line

This resource developed in partnership with the Institute of Aging is for older adults and individuals with disabilities. It provides emotional support, elder abuse reporting, well-being support, ongoing outreach calls, grief support, suicide intervention and referrals to additional services. Golden Gate Regional Center (GGRC)

This resource provides lifelong services and supports to individuals with developmental disabilities in San Mateo County. To be eligible, a member must have a developmental disability that meets the definition set forth under the Lanterman Act and originated before the age of 18. The disability must be considered “substantial” and likely to continue indefinitely.

California Children’s Services (CCS)

This resource provides diagnostic evaluations, case management, medical therapy, and funding for medical services to HPSM members under the age of 21 with physically disabling medical conditions that are eligible for care.
