HPSM Dental Providers
Choose a network provider
Select an HPSM network dentist using HPSM's Provider Directory. (If you want a printed directory mailed to you, email email@example.com.) New dentists are added to the Directory as they join the network. If your dentist is in the HPSM network of dentists, you can continue seeing them for dental care. If you looked for a dentist and could not find one, call HPSM for help. Once you have chosen a dental provider, call their office to schedule your first appointment.
Request continuity of care
If your dentist is not in the HPSM network, and you have gone to your dentist in the past 12 months, you can request continuity of care with that dentist for up to 12 months from the date of your request.
After the 12 months, you will not be able to keep your dentist unless they join the HPSM network of dentists. If your dentist does not join the HPSM network of dentists, HPSM will help you find a new dentist who is in the HPSM network of dentists.
To request continuity of care from a non-network dental provider you can do one of the following:
Email CustomerSupport@hpsm.org including “Continuity of Care Request” in the subject line. Make sure your email includes your Member ID number, and dentist information.

Medi-Cal members; Call Member Services at 1-800-750-4776 or 650-616-2133 Monday–Friday 8:00 a.m. to 6:00 p.m. TTY: 1-800-735-2929 or dial 7-1-1
